What Is Male Breast Reduction? A Complete Guide to Gynecomastia Surgery

May 28, 2025 by Nuance Cosmetic Surgery

Excess glandular tissue in the chest can make it difficult to see the effects of diet, exercise, or muscle building. Even with consistent effort, gynecomastia can leave the chest looking full and create a silhouette that doesn’t match your progress or goals. This condition can affect one or both sides of the chest and is often resistant to fat loss techniques.

Male breast reduction is an option for addressing this type of chest fullness when other methods are not enough. At Nuance Cosmetic Surgery in Folsom, CA, we offer surgical treatment plans for patients who want a flatter, more defined chest contour. While the reasons for seeking surgery vary, the goal is to remove excess tissue that will not respond to non-surgical changes.

What Is Gynecomastia and What Causes It?

Gynecomastia is the enlargement of male breast tissue due to a hormonal imbalance, most often an increase in estrogen relative to testosterone. This condition can affect newborns, adolescents, and adult men, and it may occur on one or both sides of the chest. Some patients experience tenderness or sensitivity in the area, while others notice only the visual fullness. Gynecomastia causes include puberty, aging, certain medications, anabolic steroid use, alcohol or marijuana use, thyroid or liver disorders, and obesity-related hormonal changes. In many cases, the glandular tissue does not go away on its own, especially if the condition has persisted for more than a year.

What Is Male Breast Reduction?

Male breast reduction is a surgical procedure that removes excess breast tissue and, in some cases, fat through direct excision or liposuction. We often combine both methods to address firm glandular tissue and any surrounding fat that contributes to chest fullness. This procedure gives us the ability to sculpt a flatter, more proportional chest that aligns more closely with your frame. We may also tighten the area to avoid loose skin after the tissue is removed.

Types of Tissue Removed With a Male Breast Reduction

Excess Fat

When fat is a major component of chest fullness, liposuction can be an effective method for chest fat reduction. This type of fat is usually softer and responds well to suction techniques through small incisions. We insert a thin cannula to break up and remove the fat, which allows us to sculpt the area.

Glandular Tissue

Glandular tissue is dense and firm, often located directly beneath the nipple-areola complex. This tissue does not respond to weight loss or liposuction and must be removed through direct surgical excision. We make a small incision, usually at the edge of the areola, to remove the glandular tissue.

Excess Skin

In cases where the skin has stretched, especially after significant weight loss or long-standing gynecomastia, we may need to remove or tighten loose skin during the surgery. Sagging skin can prevent the chest from looking flat or firm, even after the underlying fat and glandular tissue are removed. We assess the skin’s elasticity during your consultation to determine if skin tightening will be necessary.

The Decision to Pursue Surgery

Many patients consider gynecomastia surgery after they reach a stable weight and see little to no change in chest shape with exercise. If the tissue has been present for more than a year, it is less likely to go away on its own. Surgery becomes a practical option when glandular tissue or resistant fat limits your ability to create the chest contour you want. We assess how much tissue needs to be removed and whether any skin tightening will be necessary to reach your goals.

A Closer Look at Male Breast Reduction Surgery

Anesthesia

We typically perform male breast reduction using general anesthesia to keep you comfortable throughout the procedure. Some smaller cases, particularly those requiring only liposuction, may qualify for local anesthesia with sedation, but this depends on the treatment plan. You will discuss anesthesia options during your consultation so we can plan the safest and most effective approach.

What Is Recovery From Male Breast Reduction Like?

During gynecomastia recovery, you can expect some bruising, swelling, and tightness in the chest for the first few days. We apply a compression garment to help minimize swelling and support the new shape as it heals. Most patients return to work within a week, although strenuous activity and upper body workouts need to wait a few more weeks. We review your activity restrictions at each follow-up and gradually clear you for normal movement.

Addressing Common Concerns About Gynecomastia Surgery

Will My Gynecomastia Come Back?

Once we remove glandular tissue during surgery, it does not grow back. However, new fat can accumulate if your weight increases or hormone levels shift significantly, which may cause the chest to look fuller again. The best way to maintain the results is to keep a stable weight and avoid medications or substances that affect hormone balance.

What Happens If I Gain Muscle After Surgery?

Muscle gain after gynecomastia treatment often improves the chest contour further, as there is no longer excess tissue masking your results. Building pectoral muscles can add natural projection and shape, and give the chest a more athletic appearance. We recommend waiting until you are fully healed before resuming chest-focused strength training. Once cleared, regular workouts will not reverse the effects of surgery and can help maintain your outcome.

What Happens If I Gain Weight?

If you gain weight after surgery, fat may return to the chest area, especially if your body tends to store fat there. The glandular tissue we remove will not come back, but the remaining fat cells can still expand. Minor fluctuations usually do not affect your results, but larger changes in body composition may reduce definition. We encourage patients to maintain their weight through diet and regular exercise to preserve the flatter contour.
