Is Blepharoplasty Right for You? What to Consider Before Eyelid Surgery

April 28, 2025 by Nuance Cosmetic Surgery

Your eyes are often the first feature people notice about you. However, blepharoplasty may be worth considering if drooping eyelids or under-eye bags affect your appearance or even your vision. Also known as eyelid surgery, this popular procedure removes excess skin, fat, or muscle from the upper and/or lower eyelids to rejuvenate the eyes and enhance overall facial harmony.

While blepharoplasty is a highly effective treatment, it’s still a surgical procedure. If you’ve been wondering whether it’s the right choice for you, this guide breaks down what to know before committing to eyelid surgery.

What Is Blepharoplasty?

Blepharoplasty is an eyelid surgery that refreshes the appearance of the eyes by removing or adjusting extra skin, fat, or muscle from the eyelid area. As we age, the skin around our eyes can become thinner and less firm, leading to sagging, puffiness, or a constantly fatigued look, regardless of how rested you actually feel. In more severe cases, drooping upper lids can interfere with vision, making it harder to do daily activities like reading or driving comfortably.

There are two main types of blepharoplasty, and both can be performed alone or together, depending on your unique needs and goals:

Upper Eyelid Blepharoplasty

This procedure addresses loose or excess skin that creates hooding on the upper eyelids. It’s especially beneficial for patients whose upper lids sag so significantly that they interfere with vision. It can also smooth out puffiness caused by fatty deposits and restore a more youthful, open look to the eyes.

Lower Eyelid Blepharoplasty

This surgery focuses on the area beneath the eyes. It is commonly used to reduce the appearance of under-eye bags, correct puffiness, and remove or tighten loose, wrinkled skin. Fat is sometimes repositioned rather than removed to restore volume and smooth the transition between the lower eyelid and cheek.

Some patients opt for combination blepharoplasty, addressing both the upper and lower eyelids to achieve balanced, full-eye rejuvenation. This option can create more dramatic, natural-looking results and may also reduce the overall recovery period compared to two separate surgeries.

What Should You Consider Before Eyelid Surgery?

Choosing to have eyelid surgery is more than just a desire to look more awake or youthful—it’s a personal decision that deserves thoughtful consideration. While blepharoplasty is generally safe and delivers excellent results for many patients, it’s important to look at the bigger picture before moving ahead.

Your Goals & Expectations

Before undergoing eyelid surgery, it’s important to understand your goals clearly. Are you hoping to improve sagging upper lids, under-eye bags, puffiness, or drooping? While eyelid surgery can refresh your appearance and make you look more alert, it won’t dramatically change your face or stop aging. Having realistic expectations helps ensure satisfaction with the results.

Medical Evaluation

A thorough medical evaluation is essential before scheduling surgery. Your surgeon may recommend a complete eye exam to assess your vision, tear production, and overall eye health. In some cases, vision tests or clearance from your optometrist or ophthalmologist may be required, especially if sagging lids impair your sight.

Health Conditions

Certain health conditions can increase the risk of complications during or after surgery. If you have chronic dry eyes, thyroid eye disease, high blood pressure, diabetes, or a bleeding disorder, be sure to disclose these to your surgeon. Smoking is another major factor that can delay healing and increase the chance of poor outcomes. Most surgeons will require you to stop smoking several weeks before and after surgery.

Medications & Supplements

Certain medications and supplements can affect both your surgery and how well you heal afterward. Products like aspirin, ibuprofen, fish oil, and vitamin E can thin the blood, which may raise the risk of bleeding during or after the procedure. To stay safe and avoid complications, it’s essential to share a full list of everything you take—prescriptions, over-the-counter meds, and herbal remedies included—so your surgeon can guide you on what to pause or adjust before surgery.

Recovery Time

Recovery from eyelid surgery typically involves one to two weeks of downtime. You can expect swelling, bruising, and possibly some blurry vision during this period. You may need someone to assist you at home for the first day or two after surgery. While initial healing happens quickly, full recovery and final results can take several weeks to a few months.

Cost & Insurance

If you’re considering eyelid surgery for cosmetic reasons, keep in mind that insurance typically doesn’t cover the cost. However, if drooping eyelids affect your vision and the procedure is considered medically necessary, you might be eligible for partial or full coverage. You’ll likely need supporting medical documentation and pre-approval from your insurance provider to find out. Requesting a detailed estimate of all associated fees, including the surgeon, anesthesia, and surgical facility costs, is also a good idea.

Risks & Complications

Like any surgery, eyelid procedures come with risks. These may include dry eyes, difficulty closing your eyes, scarring, asymmetry, infection, bleeding, or, in rare cases, changes to your vision. It’s essential to have a detailed conversation with your surgeon about these potential risks so you can make an informed decision.
